Many times before bids are accepted by the District, an addendum or change in the specification is released.  Completing this form lets us know who has an incomplete or inaccurate set of specifications.

Note:  If you do not provide us with a valid e-mail address, we will not be able to respond to your request for a  proposal.  All fields are required except the fax field.
